Summer Camp Families: Please read this
Attention Summer Camp Families – PLEASE READ THIS
I have just learned of another change to the medical
form procedure for this year (don’t you just love
state bureaucracy). If your son is bringing
medication to camp, a “Medication Authorization Form”
needs to be filled out and signed by a doctor for each
medication.
This form can be downloaded on the Camp Mattatuck
Website here.
I apologize for the late notice on this form, but the
state made this ruling after all the summer camp
information was sent from Camp to us.
****If you haven’t turned in your medical form to Mr.
Lukens, please bring it to a troop meeting as soon as
you are able.****
